After examination in kerala, doctor diagonised as allergic cough and advised for a 5 months Montelukaste(4mg)treatment. After 10 days of treatment, the cough was vanished almost completely. After one month we came back to muscat and again the problem has started. Now he has finished his one course anti-biotic(Omnicef). Reference to muscat docotr, after taking the anti-biotic, chest is clear but slight wheesing is there and advised to take prednisolone 5 mg tid for 3 days. No history of Asthma in our family. Reply: Your son is is having allergy.The good response to Montelukast point towards it.The further management depends on clinical evaluation.If he is having recurrent wheezing- there is a chance of asthma.Though it is run in families and has got a genetic background, asthma can occur with out familial background.In case of asthma inhaled medications are the safe and effective medications available.Before choosing the medications you have to consult a pediatrician/pulmonologist.

Reply: It could be a Bulla.There are many reasons for developing this including smoking and congenital reasons.Dependsing upon its cause, size, efeect on lung function the treatment differs.Though it may not cause any problems, it can rupture and cause some problems at times.Hence you are adviced to consult a pulmonologist or a thoracic surgeon.

A chest X ray may show rounded or oval shaped shadows, collection of fluid etc.Ct scan is also helpful. To confirm the diagnosis Lung biopsy or needle cytology is needed.

Reply: Some dots or scratches in the X ray is a common issue.Probably it is due to the old infections including TB.Many time we may not be aware of the illness which occurred years ago.Usually this is not a problem at all.But at times these X ray shadows may create problems while going for medical examination for jobs abroad. Any way dont worry about it.If you need more clarification consult a physician with the X ray film and all other medical details.

Reply: Your father is having COPD(Chronic Obstructive Pulmonary Disease).It is mainly caused by smoking.Atmospheric pollution, work environments,Domestic gases and fumes etc can also cause this. We have effective medication for this condition.But it is not as effective as in the case of asthma.In the later stages of the disease patients has to use oxygen therapy in home to control symptoms.This should be prescribed only after detailed evaluation.periodic check ups and assessments are also needed to adjust the oxygen dose. Reply: Your history seems to be of an upper respiratory problem.But you have to rule out lung problem first.Since the X ray is normal assess your lung capacity by a test called Spirometry.If ok the difficulty is likely to be due to a throat problem.Then you can consult an ENT specialist.

Reply: Dont worry .Your son is taking the correct medications.Venrolin is not for regular use and if taking occassionally no problem.Have periodic check up so that the disease status can be assessed.Lung function can be measured by a simple breathing test called Spirometry.Other measures to control asthma apart from medications include- Avoid dusty atmospheres, extreme cold climates, pets, over use of room freshners, mosquito repellents, food additves.If any particular food is allergic it should also be avoided.Tobacco smoke is dangerous, even passive smoking.If anybody in the family smokes stop it at the earliest.

Reply: As you are a diabetic and hypertensive your symptoms can be related to those conditions.Heart problems and lung diseases can have such symptoms.The data provided by you indicate that a detailed check up is necessary in your case.My opinion is to consult a physician to evaluate the diabetic status and hypertension.If you have repeated breathlessness you may have to consult a pulmonologist for doing spirometry to assess your lung capacities.
